On October 25, 2011, the Vermont Division of Fire Safety issued the following bulletin: In August 2004 Chemetronics (Kidde-Fenwal Company) issued a bulletin indicating 500 and 600 Series heat detectors produced January 2001 through December 2003 may be defective. (date code W01-Y02 week 1, 2002 to W52-Y03 week 52, 2003).
The problem found was rivets that hold the wire attachment to device was out of tolerance on some detectors, can separate from the device causing it to be inoperative. The wires are still connected to screw terminal, however once detached from device the detector can’t transmit an alarm signal to fire alarm control panel. Because the wires are still attached to the terminals, no trouble notification is transmitted to the fire alarm control panel.

October 5, 2010 Dear Fire-Lite Alarms Customer, The US Consumer Product Safety Commission (CPSC) has recently issued a recall alert (#10-8/17) for Fire-Lite Alarm’s MS-9600UDLS fire control panel installed with a SLC-2LS Expander Module. This recall alert is a result of Fire-Lite Alarm’s Safety Bulletin FLSB10-3-1 that was distributed April 2, 2010, and is not a new issue. The CPSC has issued its own alert at this time, which is directly related to the issue communicated by our Safety Bulletin. There is no actual “recall” associated with this alert. The remedy is a free software upgrade. This Safety Bulletin is very important and requires immediate action by those who install and service Fire-Lite MS-9600UDLS series fire alarm control panels. WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of Product: Kidde Model PI2000 Dual Sensor Smoke Alarms
Units: About 94,000
Manufacturer: Walter Kidde Portable Equipment Inc., of Mebane, N.C.
Hazard: An electrostatic discharge can damage the unit, causing it not to warn consumers of a fire.
Incidents/Injuries: The firm has received two reported incidents of smoke alarm malfunctions involving electrostatic discharge during installation. No injuries have been reported.
Description: This recall involves Kidde dual sensor smoke alarms model PI2000. The alarms can be identified by two buttons, “HUSH” and “PUSH AND HOLD TO TEST WEEKLY,” which are located on the front/center of the alarm. The model number and date code are on the back of the smoke alarm. Only date codes 2008 Aug.01 through 2009 May 04 are included in this recall.
Sold at: Retail, department, and hardware stores and through electrical distributors nationwide from August 2008 through May 2009 for between $30 and $40.
Manufactured in: China
Remedy: Consumers should contact Kidde immediately to receive a free replacement smoke alarm.

The following product safety recall was voluntarily conducted by the firm in cooperation with the CPSC.
Name of Product: Notifier Fire Alarm Control Panels
Units: About 2,000
Manufacturer: Notifier, of Northford, Conn.
Hazard: The software in the fire alarm system could fail to process an alarm condition which could result in the system failing to respond in the event of a fire.
Incidents/Injuries: None reported.
Description: This recall involves the Notifier brand operating system firmware control panels in fire alarms. The control panel is black or red with a silver screen. The panels control display and emergency communications. "NOTIFIER" or "NOTIFIER by Honeywell" is printed on the front of the fire alarm unit. This recall includes the following model numbers: Firmware version 11.2.2 for NFS2-640 and NFS-320 control panels and firmware version 3.14.1 for and NFS-640 control panels.
Sold by: Authorized distributors to commercial end-users. The products were not sold at retail, but version 11.2.2 was available for download on www.magni-fire.com, a secure Web site accessible only to Notifier customers from May 2008 through July 2008 at no charge.
Manufactured in: United States
Remedy: Distributors should immediately contact the company for replacement software. All known users of have been contacted.

Underwriters Laboratories (UL) is notifying consumers, sign
installers, and Authorities Having Jurisdiction that electric signs
manufactured by Clover Signs bear a counterfeit UL Mark. Clover Signs
is not authorized to use the UL Mark on its signs or represent that its
products are UL Listed. These signs have not been evaluated for safety
by UL and are not authorized to bear the UL Mark.
Units: Unknown quantity.
Manufacturer: Clover Signs, Chicago, Illinois
Date of Manufacture: April 2005 to present
Identification: Attached to the sign is a label which has a
counterfeit UL Mark. Issue numbers BF-565351 through BF-565362,
BR-774301 through BR-774800, or CX-285601 through CX-286100 also appear
on the label. The company name CLOVER SIGNS and/or "E215710" may also
be on the label or found on a separate label.
What You Should Do: UL recommends that these signs not be put
in service. If the signs are already installed, UL recommends
contacting qualified service personnel to take the signs out of service.
